Loss of Passport


A. For Workers

1. Original Citizenship Certificate of the Worker/s
2. Company letter in detail about the incident and future precaution.
3. duly filled passport application form; preferably Pre-live Enrollment form.
4. Four passport size colour photographs (for manual application)
5. One photocopy of the lost passport (photo page and current visa page on a single sheet) to be attached with each application form.
6. Two copies of the Police Report regarding the loss and its English translation. Also provide the complete mailing address (in English), Telephone & Fax no. of the police station where the report was made.
7. One set (both hard print & soft copy) of the name-list of the workers together with their passport numbers, place of issue, permit expiry date and contract expiry date. Application forms are also to be arranged according to this list.
8. Photocopy of the approval of the Calling Visa from Immigration Malaysia. (Please highlight the relevant names and passport numbers)
9. The Immigration Department should be promptly notified of the loss / theft of the passport. Attach a copy of the same letter along with its English translation.
10. Report of the insurance company, where the company has insured company premises and the documents.
11. Photographs of the theft area, including the safe, lockers, etc. Please use full sheet of A4 size papers to paste photographs.
12. Company Boss himself/herself (or HR Manager with authorization letter from the Boss) and specified representatives of workers are to come to the Embassy for interview, along with the abovementioned documents.
13. Employment Contract between Employer and the Employee
Please Note:
1. The Embassy may also visit the company to assess the report of the loss of the passports.
2. In that case, the company should provide all the details and make available the head/HR Manager and the affected workers for interview.

B. For Students

1. Proof of enrolment at a recognised educational institution in Malaysia/Indonesia/Philippines/Brunei (School/College/University etc.)
2. Letter from the educational institution in its official letter-head to the embassy regarding loss/theft of passport.
3. One duly filled passport replacement application form preferably online form.
4. Four copies of passport size colour photographs (only with manual forms)
5. One photocopy of the lost passport (photo page and current visa page on a single sheet) to be attached with each application form.
6. Two copies of the Police Report regarding the loss and its English translation. Also provide the complete mailing address (in English), Telephone & Fax no. of the police station where the report was made.
7. The educational institution should be promptly notified of the loss / theft of the passport. Attach a copy of the same letter along with its English translation.

C. For Others

1. Nepali nationals who are visiting Malaysia/Indonesia/Philippines on a tourist visa may contact the embassy in case of loss of passport with police report, and a attested copy of the lost passport or attested copy of Nepali citizenship certificate along with the proof of purpose of the visit to the respective countries.
2. Upon review, the embassy may issue passport or travel document.
